Adult Transition Assessments

We offer specialized assessments for adolescents over the age of 16 who may be eligible for individualized adult support services (through Community Living) on the basis of an intellectual disability or a specific developmental disability, such as a diagnosed Intellectual Disability, Autism Spectrum Disorder, or Fetal Alcohol Spectrum Disorder.

Adult Transition Assessments include any of the following:

  • cognitive assessment
  • adaptive skills assessment
  • student observations
  • previous assessment documentation review
  • school documentation review
  • transition planning and funding consultation
  • pertinent documentation required by CLBC
  • direct communication with CLBC regarding eligibility
  • strengths-based recommendations
  • recommendations for interventions and support

This specialized assessment service supports the transition from school to adult life for young people with complex needs. We work closely with the individual and their family members to best understand the support needs within a future-oriented, culturally sensitive model.
